Step 1:

Complete an APPLICATION FOR ENROLLMENT

Step 2:

Complete a STUDENT BACKGROUND SURVEY FORM K-5TH or STUDENT BACKGROUND SURVEY FORM 6TH-12TH

Step 3:

Schedule a time and date for the diagnostic entrance test

Step 4:

Grades 2-12 will complete a CONFIDENTIAL CHARACTER REFERENCE FORM

Step 5:

Acceptance into the academy will be based upon the entrance interview (if required), testing results, references, behavioral history and academic profile.

Step 6:

Sign and return the STUDENT/PARENT HANDBOOK AGREEMENT
